I've done many of them.

Keep in mind, TV has been viewed at a seated eye-level for the last 50 YEARS. Going and putting above a fireplace may not be compatible with who is watching it.

Eye and neck strain can be relative to how close you are sitting to it. The closer you are, the more you are going to be tilting your neck to watch it.

I'd recommend at least 13' back from the display to watch it comfortably.

I've had people who wanted to put a 60" display on top of their fireplace and sit about 10' back from it. I told them they were crazy. Not only will the top of the TV be touching the ceiling, they would be looking at least 45degrees up.

For heat concerns, never had any providing you have a mantel. If your fireplace is flush with the wall with no mantel, then there will be more heat coming up and can cause performance issues.

Other things to worry about are, do you have an electrical outlet above the fireplace? Did you have a conduit put in in order to run all your A/V cables up there? If not, then these things will need to be put in.

Also, how is it going to make the room look? Which do you want to highlight more in the room? The fireplace or the TV? A large TV will dwarf the fireplace.
